Athletic Trainers

Kent Chapman, ATC
Athletic Training and Special Projects Program Manager 

Kent grew up in southern Wisconsin and attended the University of Wisconsin-La Crosse. He received his bachelor of science degree in athletic training and sports management in 1991. As a certified athletic trainer for 23 years, Kent has worked with a variety of athletes at the professional, collegiate, Olympic and high school levels. Since 2002, he has been in Southern Indiana working with local high schools in the prevention, evaluation and rehabilitation of injured athletes.

Kent lives in Louisville with his wife, Tamara, and their two kids, Carter and Kenna.

Sarah Chauvin, MBA, LAT, ATC, CSCS, CKTP, CEAS

Sarah joined Baptist Health Physical Therapy in 2016 and provides sports medicine coverage to many of the high schools Baptist Health covers in Southern Indiana. Sarah completed her Bachelor of Science degree in athletic training at Purdue University in 2013. Since graduation, she has been working as a special education paraprofessional at Eastern High School, as well as providing athletic training services for the school.

In her spare time, Sarah enjoys running and watching Green Bay Packers football.

Tyler Deuser, LAT, ATC

Tyler joined Baptist Health Floyd Physical Therapy in 2015. He received his bachelor of science degree in athletic training and exercise science, as well as his MBA from the University of Indianapolis. After graduating from college, Tyler was employed at St. Vincent Sports Performance Center in Indianapolis, where he trained and treated athletes of all levels from all over the United States.

Tyler has worked with professional athletes from the NFL, NBA, NHL, USA Gymnastics and Indy Car. He has also provided performance training and speed development for college athletes training for the NFL and NBA combines. His role as a wellness coordinator, company owner, orthotic fitter and athletic trainer has involved collaborating with physicians, patients, coaches, peer trainers and agents to provide medical care services to a variety of individuals.

Tyler has delivered presentations nationwide on muscle activation, dynamic warm-ups and treatment methodologies in the clinical setting. He is excited to provide athletic training coverage for Silver Creek High School in Sellersburg.

Kristie L. Dixon, ATC

Certified athletic trainer, Kristie L. Dixon, ATC, joined Baptist Health Physical Therapy in 2015 in order to help with sports medicine coverage for several Southern Indiana high schools. Currently, Kristie provides athletic training coverage for Borden and Henryville High Schools. Kristie completed her undergraduate degree at the University of Evansville in 2015. During her senior year at Evansville, she enjoyed working exclusively with their Division I baseball team.

In her spare time, Kristie enjoys going for a run, spending time with her family and friends and watching sports!

Kara Earl, ATC, EMT-B

Kara joined Baptist Health Physical Therapy in 2016 and is the athletic trainer working with South Central High School. Kara completed her first bachelor’s degree at the University of Kentucky in 2012 in Kinesiology – Exercise Science with a minor in Health Promotion. In 2016, she received her second undergraduate degree in Athletic Training from Eastern Kentucky University, and became EMT certified. While in school at EKU, Kara had the opportunity to intern with Marshall University and worked on and off the field with the Marshall football team. Throughout her athletic training studies, Kara has worked at several high schools and colleges in the state of Kentucky. In addition to athletic training, Kara has worked for University of Kentucky Hospital and Drayer Physical Therapy for four years as a physical therapy technician. She has worked in both inpatient and outpatient settings, helping patients of all diagnoses, ages and complexities.

Scott Gravatt, MS, ATC

Scott joined Baptist Health Floyd Physical Therapy in 2015 and provides athletic training for Our Lady of Providence High School. Scott is a graduate of Salem High School and completed undergraduate degrees at Emory and Henry College. He received his master’s degree in athletic training from Indiana University in 1990. Scott has provided athletic training coverage in several locations in Indiana, Ohio and Virginia. He is currently completing licensing in Kentucky.

In addition to athletic training, Scott has been a teacher for 20 years, teaching physical education at the elementary, high school and college levels, as well as driver’s education, drafting, sports medicine and advanced physical education. He has had the opportunity to work with many Division 1, 2, and 3 athletes in sports, including football, basketball, track and soccer. Scott has also assisted in presenting at local seminars and regional presentations.

Scott has previously worked in conjunction with Indianapolis-based Rock Steady Boxing, which affiliated with Reid Hospital and Health Care Services to utilize a boxing training and conditioning techniques program to help improve the quality of life for Parkinson’s patients. Scott has been working primarily with Reid Hospital’s Eaton, Ohio facility, and is excited about the possibility of beginning a similar program here in Southern Indiana.

Kimberly Holzbog, MS, ATC

Kimberly joined Baptist Health Floyd Physical Therapy in 2015, and provides athletic training services for North Harrison High School. She completed her bachelor’s degree in athletic training at Franklin College in 2014, and her master’s degree in athletic training from Indiana Wesleyan University in 2015.

Since mid-2014, Kimberly has been working with the athletes at North Harrison High School, as well as providing medical coverage for numerous other local sporting events.

In her free time, Kimberly enjoys traveling, outdoor activities and attending sporting events. 

Matthew Rice, ATC

Matthew is a lifelong resident of Southern Indiana and lives in Starlight with his wife and son. He graduated from Indiana State University in 2007 with a bachelor of science degree in athletic training. He then enhanced his education by completing a two year internship with the Western Kentucky Hilltoppers.  Since 2009, Matthew has been the sole provider of athletic training and sports medicine coverage at Charlestown High School, as well as covering a variety of sporting events throughout Southern Indiana and the Louisville metro area.

In his free time, he enjoys hunting, fishing and spending time with his family.
